The eLAJ Smart Clinic: Unlocking Digital Innovation in Primary Care

This case examines the evolution of the Biocon Foundation’s eLAJ initiative as a model of digital health innovation and social entrepreneurship in India’s primary healthcare system. Emerging from the Foundation’s earlier Arogya Raksha Yojana (ARY) micro-insurance scheme, eLAJ evolved into a technology-enabled primary healthcare model focused on non-communicable diseases (NCDs), continuity of care and healthcare access for underserved populations. Guided by the vision of Kiran Mazumdar-Shaw and led by Dr. Anupama Shetty, the initiative reflected a shift from compliance-driven Corporate Social Responsibility (CSR) toward strategic health system innovation.

Set against India’s pursuit of Sustainable Development Goal 3 (SDG 3) and Universal Health Coverage (UHC), the case highlights persistent healthcare challenges including low public health expenditure, shortage of health workers, fragmented digital systems, and rising NCD burdens in rural, tribal and urban vulnerable populations. eLAJ addressed these gaps through electronic medical records, diagnostics, preventive screening, community outreach, and partnerships with Primary Health Centres (PHCs) in Karnataka and Rajasthan. The initiative improved healthcare utilization, reduced out-of-pocket expenditure and strengthened continuity of care. The case also explores the opportunities and uncertainties of scaling digital health innovations through public-private partnerships and social entrepreneurship. Attention is given to workforce management, patient-centred care and the challenges of integrating digital systems into resource-constrained settings. Students are encouraged to evaluate trade-offs between cost, access and quality, while examining issues of leadership, sustainability, governance and digital transformation in healthcare delivery.
